Parent or Guardian agree to the following:

An Enrollment Form is required for all students participating in this program. Contracts will be billed 50% of rate prior to the first session and 50% of rate on January 1st. Drop-in charges and late fees will be billed monthly. Payments are due upon receipt. A late fee will be assessed at a rate of 1.5% per month on all accounts past due. Delinquent accounts may result in dismissal of student from the Junior Pitch Deck (JPD) Program. Please note that contract changes can only be made prior to the first session & before the start of the January session. The After School Program ends promptly at 12 p.m. After 12:00 p.m., parents will have a grace period of 15 minutes for pick up and after 12:15 p.m. a late charge will be assessed at the rate of $10.00 for each 10-minute interval or fraction thereof.

Only persons authorized by the parent or guardian may pick up your child from the After School program. Please notify the Director in writing via email if anyone other than a parent/guardian will pick up your child. All students attending the JPD Program are required to follow all program rules. Behavioral problems will result in parent notification and possible suspension from the program.

No Transferability, No Exchanges, No Refunds, and No Make-up classes are permitted once enrolled. If JPD needs to reschedule, there will be notice for a rescheduling date.

Use of Student’s Likeness Waiver. I, the Parent, authorizes and agrees for Junior Pitch Deck and its affiliates to use and irrevocably grants Junior Pitch Deck property rights in of any and all of Student’s likeness, images, voice, and testimonials whether electronic or in writing, including but not limited to videos, photographs, voice recordings, text messages, provided or derived from an interaction with Junior Pitch Deck, and/or that relates to the class, for use in the business of Junior Pitch Deck. Student nor the Student’s family or others related to Student are entitled to compensation of any kind for Junior Pitch Deck’s use.

Indemnity. Parent AGREES TO DEFEND, INDEMNIFY, AND HOLD HARMLESS THE JUNIOR PITCH DECK (JPD), JPD EMPLOYEES, MEMBERS, OFFICERS, AGENTS, REPRESENTATIVES, CONTRACTORS, FAMILY PROMISE, FAMILY PROMISE MEMBERS, OFFICERS, AGENTS, REPRESENTATIVES, AND CONTRACTORS FROM AND AGAINST ANY AND ALL CLAIMS, LOSSES, DAMAGES, COMPLAINTS, DEMANDS,OBLIGATIONS, ACTIONS,
LAWSUITS, JUDGMENTS, AWARDS, PENALTIES, VERDICTS, OR CAUSES OF ACTION OF ANY KIND, INCLUDING BUT NOT LIMITED TO CLAIMS FOR
PERSONAL INJURY, DAMAGES TO A BUSINESS, CONTRACT, TORT, CONTRIBUTION AND/OR INDEMNITY, NEGLIGENCE, AND ANY OTHER LIABILITY OF ANY KIND BROUGHT BY PARENT OR ANY THIRD PARTY RELATED TO OR ARISING OUT OF THE AGREEMENT OR PRESENCE AT FAMILY PROMISE.
